Seamless Transition from Hospital to Home:Ms. Wu’s Three-Week Recovery Care
Ms. Wu is a 78-year-old woman living near the China Town. Due to a fall that caused a hip fracture, she underwent surgery and required three weeks of post-operative recovery care after discharge. Her family hoped that she could recover comfortably at home. After assessing her condition, we arranged a suitable home care plan for her. We scheduled 4 hours of home care per day, mainly assisting with getting in and out of bed, personal hygiene, meal preparation, and accompanying her for outdoor walks. We also provided Cantonese-speaking PSW services to meet her needs.
After two weeks, Ms. Wu was able to walk independently with the help of a walker, gradually reducing the number of home care service hours while smoothly transitioning through her post-surgery recovery period.
